Getting a referral to our practice
A referral is a recommendation from your primary care provider (PCP) or another physician for you to be seen by a specialist. Many insurance plans, including most HMO and some Medicare Advantage plans, require a valid referral on file before they will cover a visit with a rheumatologist.
How to get a referral
- 1 Let your primary care provider know you would like to be seen at Permian Arthritis & Rheumatology Center.
- 2 Ask their office to send the referral to us by fax or electronically. Call us at (432) 326-1939 and we'll provide the details they need.
- 3 Have them include recent, relevant records such as lab work, imaging, and office notes.
- 4 Check with your insurance about whether a referral and/or prior authorization is required for specialist care.
- 5 Once we receive a valid referral, our team will call you to schedule your appointment.
What makes a referral valid
- Patient's full name and date of birth
- Referring provider's name, contact information, and NPI
- Date of the referral and a current (non-expired) validity period
- The specialty being referred to (Rheumatology)
- Reason for the referral or working diagnosis
- Insurance information and any authorization number the plan requires
- Number of visits authorized, if your plan specifies a limit

Please arrive 15–20 minutes early to complete check-in. Your first visit typically lasts 45–60 minutes.

- We accept Medicare and most major plans, including Blue Cross Blue Shield, UnitedHealthcare, Humana, Cigna, and Aetna
- Telehealth visits are available — ask if you're eligible
